Dear Parents/Guardians:

                        MEDICATIONS:  IN SCHOOL

(Guidelines set by the NJ State Department of Education)

Medications should be administered at home whenever possible.  Medications should be administered in school
only when necessary for the health and safety of our students. 

The school policy does not allow children to have mediations in the classroom unless the child has permission to
self administer life saving medication (see “permission for self-administration of medication for potentially
life-threatening illness”).  Medications will be kept in the nurse’s office and dispensed by the nurse at the appropriate
time if needed during the school day.

For all medications:

1.      The Parent/Guardian must provide a written medical practitioner’s order for the administration of over-the-counter
or prescription medication. 
The order must include: 

a.      Name of medication and diagnosis

b.      Dosage, route, and frequency

c.       Medical practitioner’s signature

2.      All medications must be in the original pharmacy container with the appropriate labeling.  Ask your pharmacist for
the medication to be divided into two bottles if necessary.  Do not send medications in bags, tissues, envelopes, etc. 
It is against the law to dispense unlabeled medications.

3.      Students are not allowed to carry medications into school for safety reasons.  An adult is the preferred person to drop
off medication in the main office or school nurse’s office

This permission form is effective for the school year for which it is granted and must be renewed each school year.
